According to the wording in the rules an agent may be promoted up to three levels for a successful mission.
Does that mean up to three levels for one mission or that 3 is the ceiling and that after 3 successful missions the agent can't improve based on mission success?

No, what that means is that while you may only be able to train an agent to L12, he can possibly achive L15 with successful missions. Maximum of one level per mission.

Frequently they will not be promoted though.
Even if you have a successful mission there's a chance they will/wont be promoted.

So the promotion only applies to agents at the limit of your training levels or is it ongoing at all levels until you get to limit+3?

It's ongoing and can happen before your agent is maxed out training wise. So, suppose you kidnap a noble or something... if you succeed there's a chance your guy will be promoted doing the mission.
if you're completely maxed out though then NO chance.
