Pro tip: You can sleep a freshly captured prisoner to ensure the execute occurs. How odd is that?
Another one: You can counter-espionage at your capital to reduce the chance for a prisoner to escape (but if the escape succeeds the agent is lost).

She's in a cell or dungeon in the PC she tried to Rebel or Usurp. You get her back by having her Attempt to Escape (#385). Note several things about Attempting to Escape:
1. Base chance is 15% but several kingdoms have advantages here, like the Rangers for example;
2. You can attempt the order several times. It consumes the order against your order limitation, but only a successful Escape counts as the sole order for the emissary.
3. See order #385 in that you can also enhance the chance of escape by offering a bribe to the guards.
You'll want to Attempt to Escape the first turn after capture in case your captor Orders the Execution of a Hostage at #430.
